The Seventh Annual as Golf Tournament will be held at the Meadowbrook Country Club - Tulsa, OK,
On Monday, April 25th. Fee: $110.00 per person or $440.00 per foursome Includes:
● Golf Cart
● Greens Fees
● Breakfast
● Awards Reception
● Team Prizes
All registrations must be received by April 13th, 2011. Individual golfers will be assigned a team by the golf committee, NO CHANGES WILL BE MADE AT THE GOLF COURSE THE DAY OF PLAY. THERE WILL BE NO REGISTRATION AT THE GOLF COURSE, YOU MUST REGISTER AS AN INDIVIDUAL OR TEAM BY APRIL 13th, 2011. PARING OF TEAMS AND INDIVIDUALS WILL BE FINAL. NO TEAM OR INDIVIDUAL WHO HAS NOT PRE-REGISTERED WILL BE ADDED AFTER APRIL 13th, 2011. If a golfer who is registered can not attend, a substitute golfer may take the place of the original golfer, but the team assignment WILL NOT CHANGE. Entry fees will not be refunded for any reason other than closing of the course by the PGA Golf Professional at Meadowbrook Country Club.
